Athletic Participation Forms
All students interested in participating in athletics at PA will need to complete and return the following forms to the athletic office: Sports Physical Form, Parental Consent Form, Sudden Cardiac Arrest Form, and the Concussion Protocol Form, and the Athletic Handbook Form.
We offer flag football, soccer and basketball leagues for elementary grades!
